We must be the certificate holder on the insurance document, and our address must be on there as well. 

There must be a signature at the at the bottom right hand corner of the certificate or the name of the representative typed. 

Make sure that the name on the certificate is the same as the name in the computer. Non-bonded contractors don't have to exactly match, but bonded contractors need to match exactly. 

Check to see if the insurance company is the same one that is in the computer, if it not, change it to the one that's on the certificate. Also check the policy number. If it is different, change it as well. 

When entering the insurance, also make sure that there is a letter A, B, C, D, on the left hand side, designating which insurance company they are going through. 

Be sure to check that the limits of liability are correct on the certificate of insurance for each type of license.
